Home and Community-Based Services for Individuals with Developmental Disabilities

We provide services to help people of all ages who have developmental disabilities. These services are intended to maximize independence as participants live, work and socialize in their communities.

Day Services
Provides social activities, supervision, support, focus on finding a job and increasing skills needed to work in the community. Services include prevocational, supported employment, habilitative workshop, and habilitative community inclusion. 

Assistive Technology
Provides equipment to increase daily skills and independence.

Child Day Habilitation
Provides children with teaching and support in a community setting.

Community Integration/Connections
Helps a person access their community.

Consultative Assessment
For adults provides assessment and recommendations for challenging behaviors.

Day Supports
Provides activities and teaching in a provider location.

Environmental Modification Assessment
Determines home or vehicle changes to meet your needs.

Home and Vehicle Modifications
To your private residence or primary vehicle to meet your needs, stay safe, and do things yourself.

In-Home Residential Habilitation
Provides teaching in your own home (Independent Living) or family home (Supported Family Living).

Prevocational
Teaches general job-related skills needed to get a job.

Residential Habilitation
In a provider setting; includes Continuous Home, Host Home, and Shared Living.

Respite
Provides relief for your usual caregiver.

Personal Emergency Response System (PERS)
Devices to call for help.

Specialized In-Home Residential Habilitation
For adults temporarily unable to leave their home for day services; includes Behavioral and Medical.

Therapeutic Residential Habilitation
Is an all-inclusive service in a provider setting, available when you have both DD and severe mental illness.

Transportation
Provides rides to places in your community.
